Implementing a care plan for a patient simply depicts a better way for rendering care towards them. As a health care provider, such process or rendering care must be given properly in order to avoid any anomalies or accidents in terms of taking care of a patient. This gives the act of a health care provider about such responsibilities that they have to take care of.

As a matter of fact, nurses has what they call ADPIE. It is a process for assessment, nursing diagnosis, planning, implementation, and evaluation. This process have always been a useful tool among health care practitioners.

In assessment, is commonly known as data collection in which nurses tend to assess the patient about its medical history and medical examination. On the other hand, nursing diagnosis is a part where nurses tend to identify and determine the high risk factors and problems in order to formulate a nursing diagnosis. Then, after they tend to identify, nurses must seek orders from the patients physician and also collaborate with the members or staff of the hospital.

After all the assessment and nursing diagnosis, there is what we call planning in which it literally implies certain set of priorities. It is also the part where nurses develop written outcome on the patients condition. It is also the part where the nurse formuate the proper interventions to e given towards the patient.

Next, is what we call implementation. It is where health care provider must meet phtyscal and emotional needs of the patient. must be able to  provide patients safety and apply modified nursing interventions.

Last but not the least, it is the evaluation. It  is the part of the process where the documents are seen and the outcomes are revised if goals have been met. It is also where the health care  provider continues the care of the patient and refer from the community based agency.

Hence, we can truly tell that ADPIE is such a great and useful tool in order for the health care providers to render care in a holistic way. To promote a smart, efficient and develop the patients rapport and maintain safety.
